I figured anything before season 2 was probably coincidence. (Television writers generally don't have the creative freedom to create that kind of long endgame.) But the stuff that is in season 2 is very revealing.
Notably, even early on the focus is on Kurt and in the one scene that has a perfect opening for Karofsky to display some hyper-hetero behavior, in which Rachel dresses like Britney, Azimio is the only one to show any real interest. Karofsky just sort of tags on a generic follow-up to the noise while outwardly showing all the interest of a kid presented with a slice of lukewarm pizza. I think at that point, Adler had been told the direction they were going to take Karofsky this season and he did a wonderful job of setting the stage for the reveal without substantially altering what we already knew about his character. Kudos, Adler.
